Chinese mariner travels thousands of miles back home for finger salvage

Following a sudden accident during offshore operations, a mariner sustained simultaneous severe trauma to the blood vessels, tendons, and bones of four fingers. After receiving basic bandaging overseas, he raced thousands of miles back to China for medical care, and ultimately had his fingers successfully salvaged.

A 33-year-old ocean-going mariner, Mr. Zhang (a pseudonym), had his left hand crushed by a mooring cable while in Qatar. The crush injury resulted in phalangeal fractures, along with neurovascular and tendon damage to four of his fingers. The local hospital recommended digital amputation. Seeking the best possible care, Mr. Zhang made the immediate decision to return to China for treatment. Arriving at Guangzhou Heping Orthopedic Hospitalby the following evening, he underwent a highly complex microsurgical reconstructionovernight, which successfully salvaged all of his injured fingers.

At 6:00 AM on June 24th, while performing offshore operational duties in Qatar, Mr. Zhang suffered a sudden crush injury to his left hand from a mooring cable. In an instant, his left index, middle, ring, and little fingers were left bloody and severely mangled, causing him excruciating pain. The local hospital was only equipped to apply basic bandages to arrest the bleeding, but was unable to further manage the four severely traumatized digits.

The team led by Dr. Chen Longfeng at Guangzhou Heping Orthopedic Hospital immediately devised a surgical plan and initiated the operation. Intraoperatively, the surgeons explored the joints, blood vessels, and nerves of the four digits; performed a thorough debridement of devitalized tissue from the wound; and achieved reduction and fixation of multiple fractures and joint dislocations. The surgical team meticulously isolated superficial veins from Mr. Zhang's forearm one by one, harvesting four vein grafts to bridge the defects in the digital arteries via precise microsurgical anastomosis. Because excessive skin tension on the dorsum of the hand precluded primary closure, the team utilized bone cement to cover the wound. The moment the tourniquet was released, the distal tips of all four fingers regained a healthy pink hue.

Postoperatively, Mr. Zhang's left index, middle, ring, and little fingers exhibited a healthy pink color, normal skin temperature, and brisk capillary refill. All four digits were successfully saved!

33 岁远洋水手张先生(化姓)在卡塔尔时被缆绳挤压致左手四指指骨骨折、血管神经肌腱损伤,当地医院建议截指。为寻求最好的治疗,张先生当即决定回国治疗,于第二天傍晚抵达广州和平骨科医院,连夜进行了高难度显微修复手术,成功保住了全部手指。

6月24日清晨6时,张先生正在卡塔尔执行远洋工作任务,工作时左手突然遭到作业缆绳挤压,一瞬间左手食、中、环、小指,血肉模糊,剧痛难忍。当地医院仅能包扎止血,无法处理严重受伤的四指。

广州和平骨科医院陈龙峰主任团队立刻为张先生制定手术方案,并进行手术。术中,医生对四指的关节、血管、神经进行了探查,彻底清除创面失活组织,对多处骨折和关节脱位进行复位固定。团队从张先生前臂逐一寻找浅表静脉,移植4条静脉桥接缺损的指动脉,在显微镜下精准吻合。由于手背皮肤张力过大难以一期闭合,团队用骨水泥覆盖创面。松开止血带的那一刻——四指末梢转红润。

术后,张先生左示中环小指指体颜色红润,皮温正常,毛细血管反应良好。四根手指,保住了!
