Guangdong medical team heads to Fiji to help tackle high incidence of diabetes | South Pacific Medical Aid

Since September, a medical team from Guangdong Provincial People's Hospital has been in Fiji for a three-month medical cooperation program. Dr. Wang Long, an attending physician in the Department of Endocrinology, said the team hopes that they can genuinely improve Fiji's current situation of high incidence of diabetes and hyperglycemia through this assistance and in-depth exchanges.

Wang Long recalled being deeply affected by the scene on his first ward round in Fiji. "The entire ward was filled with amputees who had lost their limbs because of severe diabetic foot infections. Many had limbs removed up to the knee or even the thigh," he said.

In hospitals in Fiji, it is common to see patients on crutches or in wheelchairs. Wang explains that this is not only due to local dietary habits of preferring high-starch foods and sugary beverages, but is also closely related to the local challenges in the tiered diagnosis and treatment system. When patients are transferred from primary care, they've often developed serious complications and missed the best time for intervention.

Over the past two months, Wang's team has introduced China's experience in diabetes management into local practice in Fiji through joint outpatient clinics and departmental training. They have promoted new technologies such as continuous glucose monitoring, carried out education on dietary patterns, and worked to dispel local misunderstandings about insulin. "Many people believe that using insulin means you're at the end of life, which leads to delays in treatment. We want them to understand that timely use of insulin is a way to protect their health," Wang added.

Looking ahead, Wang hopes to move the management of chronic diseases in Fiji further upstream, fully leveraging Fiji's universal free healthcare. He thought that if they could promote routine health check-ups and assessments among residents, they could detect and control chronic diseases early, thereby reducing the heavy burden that diabetes places on the nation.

今年9月起,广东省人民医院医疗队赴斐济开展为期三个月的医疗合作。内分泌科主治医师王龙表示,团队希望通过此次援助与深入交流,切实改善斐济当前糖尿病与高血糖高发的现状。

王龙对其初次查房时的场景影响深刻:“整整一个病房都是因糖尿病足严重感染而截肢的患者,很多人被截到膝盖甚至大腿。”

在斐济医院,常常可见拄拐杖、坐轮椅的病人。王龙分析,这既源于当地人喜爱高淀粉食物和含糖饮料的饮食习惯,更与当地分级诊疗体系的困境密切相关。“患者从基层转诊上来时,往往已出现严重并发症,错过了最佳干预期。”

过去两个月,王龙团队通过联合门诊和科室培训,将中国的糖尿病管理经验引入斐济诊疗实践。他们推广动态血糖监测等新技术,开展饮食结构宣教,并努力消除当地居民对胰岛素的误解。“很多人认为打胰岛素意味着生命末期,这导致治疗延误。我们要让他们明白,适时启用胰岛素是一种保护。”

展望未来,王龙希望推动斐济的慢病管理更加前置,充分利用其全民免费医疗的体制优势。“如果能推动居民定期检查和评估,就能早期发现并控制慢性疾病,从而减轻糖尿病给国家带来的沉重负担。”
